Clinical evaluation revealed a conscious patient with normal vitals, pupil, and fundus with right facial paresis and hemiparesis (2/5 power). The clinical picture was suggestive of cortical venous thrombosis. Magnetic Susceptibility Effect in Diagnosis of Isolated Cortical Venous Thrombosis

Teaching neuroImages: Magnetic resonance susceptibility effect for acute isolated cortical vein thrombosis.

A 38-year-old woman using hormonal contraception presented with right-sided abdominal and arm clonic seizures, right hemiparesis, hypesthesia, and nausea. She denied headache. D-dimers were 350 ng/mL (laboratory reference #500 ng/mL). CT showed a small left parietal hemorrhagic infarct with adjacent hyperdense cortical vein (figure 1). MRI 15 hours after onset showed magnetic susceptibility eff...

Susceptibility-weighted imaging in the diagnosis of isolated cortical vein thrombosis.

globin serves as an intrinsic contrast agent for cerebral veins which contain deoxygenated blood. Other extravascular blood degradation products with paramagnetic properties such as methemoglobin, hemosiderin and ferritin also create a contrast in SWI. In this sequence in order to increase the visualization of the smaller veins, a phase mask obtained from the magnetic resonance (MR) phase image...

Cortical Vein Thrombosis as a Mimic for Isolated Cortical Subarachnoid Hemorrhage and Transient Ischemic Attack

Isolated cortical subarachnoid hemorrhage is rare and poorly understood. Differential diagnoses and proposed pathophysiology vary widely and the diagnostic work-up for these patients who present with transient ischemic attack-like episodes and characteristic imaging findings is still unclear. We report a case of isolated subarachnoid hemorrhage and transient neurologic deficits due to isolated ...

MR imaging features of isolated cortical vein thrombosis: diagnosis and follow-up.

BACKGROUND AND PURPOSE To our knowledge, very few MR imaging data have been reported in isolated cortical venous thrombosis (ICoVT). The purpose of this study was to describe MR imaging features, including T2*gradient-echo (GE) sequence, in presumed ICoVT. MATERIALS AND METHODS MR imaging examinations were performed in 8 patients with ICoVT (MR venography was performed in all patients and dig...

Isolated Superficial Sylvian Vein Thrombosis with Long Cord Sign: Case Report and Review of the Literature

Isolated cortical vein thrombosis (ICVT) is extremely rare. Only single case or small series of ICVT have been reported; clinical details are still uncertain. We report a case of isolated superficial sylvian vein thrombosis with exceedingly long cord sign. A 14-year-old female with severe sudden onset headache visited our hospital. Fluid attenuated inversion recovery and echo-planar T2(*) susce...
